• MEMBERSHIP IS NOT REQUIRED

• Payment is required at the time of registration. Clinic fees will be auto drafted from your Portal account using the primary payment method provided.
• Unpaid registrations will be dropped.
• Registration is non-refundable.
• A minimum of 10 participants must be registered. We will cancel the clinic and refund registration fees if we don’t have enough participants.
